Cementless THRA using 2nd Generation Metal on Metal Prosthesis Midterm (5-10years) Follow Up Study

Abstract
|
|
|
Purpose: The purpose of this study was to evaluate the midterm (minimum 5years) results of THR with 2nd generation Metal on Metal prosthesis.
Materials and Methods: Sixty cases (52 patients) of THR with 2nd generation Metal on Metal prostheses were evaluated after an average follow-up duration of 78.4 months. The age of the patients at the time of operation ranged from 25 to 74 years (average 54.7). There were 33 male patients and 19 female. Preoperative diagnosis was avascular necrosis in 36 cases, osteoarthritis in 18, hip fracture in 3, ankylosing spondylitis in 2, and tuberculosis in 1. Postoperative follow up was 6 months, and 1, 2, 3, and 4 years, and at last follow up. Physical examination, X-ray, blood and urine were collected at every follow up to evaluate the possibility of teratogenicity.
Results: Hams hip score, radiologic evaluation, Co-Cr concentration, and carcinogenic concerns were evaluated. Harris hip score improved from 49.3 points before operation to 93.2 points at last follow up. Radiologic findings revealed no need for revision of osteolysis or wear. Although Co-Cr concentration was elevated, clinically definite toxicity was not detected. Among the follow up patients, lymphatic or hematopoietic cancer was not detected.
Conclusion: We revealed excellent resistance about loosening and osteolysis at the minimum 5 years follow up results of cementless total hip replacement arthroplasty with 2nd generation Metal on Metal prosthesis. And there was no continuous accumulation of Co-Cr concentration in body and carcinogenic risk. But more long term follow up was needed about the effect of metal on human body
